A comprehensive overview of the existing strategies and of the availability of support services, vocational and educational training programmes, resources and practical tools for people with chronic disease and mental disorders in Europe will be performed. PATHWAYS consortium has agreed to focus on six main broad categories: cancers, spinal cord disorders, respiratory diseases, cardiovascular diseases, metabolic disorders, mental and neurological disorders. The users receive different services whose main characteristics will be analyzed, taking into account the different social welfare and healthcare models in Europe.
